This form is used for requesting a resolution to issues or complaints encountered by employees, as defined in the Whistleblowing  Policy.

All employees can reporting with anonymous disclosures (hide identity). 

However, the institution encourages all employees disclosures the identity to convenience and accurate to investigate.

Noted: Whistleblowers that declared identity will be received notify on the decision  

          of their disclosure from their reporting channel, as below detail:

• Level 1: 07 working day, Line Manager
• Level 2: 15 working day, Whistle Blowing Committee
• Level 3: 30 working day, Board of Director or Independent Board of Director

If person subject to investigate is a member of senior management or compliance,  you can report to: Board of Director/Independent Board of Director.
